The Illinois Department of Public Health
The Illinois Department of Public Health is the state agency responsible for enforcing state and federal nursing home laws. If you believe a nursing home is violating the law, you can file a complaint by calling 1-800-252-4343. We suggest you read Chapter 13 of SMARTCARE before calling Public Health.
If you know of an example of particularly bad care, or if you are not satisfied with the Public Health's response to a complaint, you can contact Toni Colon, the Deputy Director of Health Care Regulation for the Illinois Department of Public Health. Don't be afraid to do this: we promise you that nursing homes do it all the time. She can be reached at her Chicago office (312-793-7325,) or in Springfield (217-524-8359.) Her e-mail address is toinette.colon@illinois.gov
You can also communicate with the Director of the Department, Dr LaMar Hasbrouck, at his Chicago office (312-814-5278,) or in Springfield (217-782-4977.)
